How to get your At-Home COVID-19 Test for Free

Beginning January 15, 2022, Community First University Family Care Plan (UFCP), University Community Care Plan (UCCP), and Commercial HMO Members will be able to purchase over-the-counter (at-home) COVID-19 tests authorized by the U.S Food and Drug Administration (FDA) at no cost. Members should get their at-home COVID-19 test kits at a Community First network pharmacy counter to ensure that the kits are processed properly and that Members have no out-of-pocket costs at the point of sale.

If you purchase an over-the-counter COVID-19 test from a pharmacy, store, or online and are charged for your test, you can submit a claim to reimbursed by Navitus, your pharmacy benefits provider. Keep your receipt and complete the Navitus OTC-COVID 19 At Home Test Claim Form.

 

COVID-19 test kits will be covered at a reimbursement rate of up to $12 per test, with a limit of eight tests per covered Member every 30 days.
