Community First Health Plans, Inc., the only locally owned and managed nonprofit health maintenance organization in the Bexar County service area, invites physicians and providers to apply to join our Commercial HMO Network. To apply, please submit your request in writing to the above address or fill out a Letter of Interest on our website at CommunityFirstHealthPlans.com/Providers.

Physicians and providers in the geographic service area must meet Community First’s practice standards, qualifications, and other participatory criteria. Community First’s current service area includes the following Texas counties: Bandera, Bexar, Comal, Guadalupe, Kendall, Kerr, Medina, and Wilson. Community First is
sponsored by the Bexar County Hospital District, d/b/a University Health.

Pursuant to Subsection 11.1402 of the Texas Administrative Code and §843.305 of the Texas Insurance Code, this serves as a public notice of the 20-day application period from January 2, 2022 through January 23, 2022 during which Community First Health Plans, Inc. invites physicians and providers in the geographic service area to apply to participate in pro- viding health care services or medical care under the terms and conditions established by Community First for the pro- vision of such services and the designation of such physicians and providers.
