You can request a printed Provider Directory to be mailed to you, free of charge, by calling Member Services.
Community First Health Plans does not require a referral to see a specialist. However, some specialists may require a referral from your primary care provider (PCP) before they will see you. Additionally, some health care services may require authorization from Community First before you get care. Your PCP will take care of an authorization request for you. Call Member Services or login to the Member Portal to find out if a certain service requires authorization.
Some services that do not require a referral:
- Behavioral health services
- OB/GYN services
- Vision exams from an optometrist
