NOTICE TO PHYSICIANS AND PROVIDERS

Community First Health Plans, Inc. is accepting applications from physicians and providers who wish to participate in its commercial HMO network.

Applicants must meet the practice standards, qualifications, and other participatory criteria established by Community First. Providers can request network participation in writing or by completing a Letter of Interest online at CommunityFirstHealthPlans.com/Providers. Providers can apply at any time.

The Community First service area includes Atascosa, Bandera, Bexar, Comal, Guadalupe, Kendall, Medina, and Wilson counties. Community First is its service area’s only locally owned and managed non-profit health maintenance organization and is sponsored by the Bexar County Hospital District, d/b/a University Health.
