Please instruct your patients who are insured by Community First and have special health care needs to contact their assigned Service Coordinator directly or call 1-855-607-7827 for assistance with the following:
- Replacing necessary medical equipment, services, or supplies.
- Getting medications/prescriptions
- Finding community resources that can provide access to food or shelter
- Scheduling transportation to a medical appointment or pharmacy
Patients who do not have a Service Coordinator can call Community First Member Services at 1-800-434-2347.
For Provider assistance, please call Community First Provider Services at 210-358-6030.
Reminder: HHSC requires long-term care providers such as nursing facilities, assisted living facilities, hospices, and state-regulated group homes to regularly prepare for natural disasters. HHSC issues guidance to providers about updating their plans and encourages them to visit the Department of State Health Services Texas Ready website for emergency preparedness resources, including sample plans, supply checklists, and other information.
