Healthy Lifestyle Management Program 

Healthy Living is a health program geared toward Members who want to achieve and maintain a healthy lifestyle. Lasting success isn’t achieved through short-term changes. It’s achieved through a lifestyle change that includes healthy eating, exercise, reducing stress, and paying attention to your mental health.

Healthy Living provides Members with the following information and resources:

  • Information about weight management, healthy eating habits, and ways to stay active
  • Decision-making tools to use in your home and discuss with your Provider
  • Assistance from a Community First Care Manager for Members with severe health concerns
  • A list of community resources offering nutrition, exercise, and mental health programs
  • Monthly general health education, such as sun exposure and pool safety

Healthy Living is designed to help you understand how to incorporate healthy habits into your everyday life that can last a lifetime, thus reducing hospitalization or emergency room visits and improving your quality of life.

Virtual Zumba

Shake the day off and give your mind a dance break! Community First offers Members of all ages no-cost, virtual Zumba classes via Zoom. View our class schedule for kids and adults at the link below.

 

Meal Planning

Community First’s Meal Planning sheets, available in both English and Spanish, are a valuable tool to help you on your journey to better health. Our Meal Planning sheets include breakfast, lunch, and dinner for all seven days of the week. Just print and fill in your meals. By planning ahead, shopping for the specific items you need for each meal, and really sticking to your plan, you’ll save time and money, reduce stress, waste less, and be on the way to a healthier you.

This time, try something different. YES QUIT can help you become an expert in living tobacco-free. YMCA Weight Loss Program 

As a proud partner of the Greater YMCA of San Antonio, Community First invites Members to enter the virtual, no-cost Y Weight Loss Program: 16 Weeks to Wellness. Open to individuals, teens, and families interested in improving their nutritional habits and making healthy choices. Sign up at the link below.

Community First Members may be eligible to receive a complimentary 4-month YMCA membership for two adults and up to 4 children through participation in this program. Please contact a Community First Health Educator at the email or phone number listed below to see if you qualify.

    Body Mass Index (BMI) Calculator

    What does “healthy weight” mean to you? A good way to find out if your weight is healthy or not is by calculating your Body Mass Index (BMI). Learn more about why a healthy BMI is important and use the CDC calculator(s) below to quickly and easily find your/your child’s BMI.
